As patients enter the Reuben J. Plant Building they can see Dr. Dieguez's office straight ahead, located in Suite 4, only a short distance away at the end of the hall.

Current and former patients may request appointments by calling the office during office hours, or for your added convenience, by clicking here to send an e-mail anytime, day or night. When sending an e-mail, please be sure to include your full name (as we have it in our records), your daytime telephone number, along with the preferred date and time you wish to see Dr. Dieguez. We will do our best to accommodate your request. By the next business day, we will contact you to arrange your appointment.

Prospective patients may request a Prospective Patient Packet--the first step in scheduling an appointment with a new patient--by calling the office during office hours, or for your added convenience, by clicking here to send an e-mail anytime, day or night. When sending an e-mail, please be sure to include your full name, your daytime telephone number, along with your complete mailing address, including zip code.

Requests for packets received during office hours will be mailed that same day via the United States Postal Service. Requests for packets received after hours will be mailed the next business day. Once you have completed the packet, and mailed it to our office in the stamped, addressed envelope provided, Dr. Dieguez will review the information you have supplied and, as soon as possible, we will contact you either by phone or e-mail.
